Case: A 75-year-old man presented to his dermatologist with what was previously believed to be a 2.4 cm "cyst" on his left ear and associated pruitus.

History. Medical history includes smoldering myeloma, hypertension, and rosacea. The patient was prescribed losartan and metoprolol for hypertension and topical ivermectin following the use of doxycycline for rosacea.

Hint. Radiation therapy of 54 gy in 22 fractions was started 3 months after diagnosis and was well-tolerated.
